Codex Cleanup - Function Examples

I've noticed alot of discrepancy with the Function Examples in the Codex and wanted to clean it up.

I'll be self-assigning this ticket and using to track changes and establish a consistent format for these codex pages.

Notes while I work;

There's several different layouts, some of the more populated layouts to use when we decide on a standard;

I found alot of articles reference other functions. It would be nice if those references were linked to actual codex pages.

Any of the codex with 'Top' links seem to be sporadically broken as they have full links rather than just hash links and in some cases those links go to different pages rather than the current.

Will need to move this article into the Action Hook reference;

The function documentation is going to be automatically generated soon, and probably live on a new site. So that should take care of this.
