Codex: Reveal hidden breadcrumbs

Description

The breadcrumbs in the internal pages of the codex have been hidden by the #wpadminbar moved below the header of the site.

Attached patch addresses the issue as well remove extra border at the footer of the page.

Attached image shows the before and after screenshots.
