Group membership request form fails to load in bp-default and related themes

Description

Using the bp-default theme with BP 1.7.2 or 1.8-beta1, the group membership request form within a single group isn't loaded. If I use Twenty Twelve and rely on theme compatibility, the form is loaded as expected.

Looking at the generated html, I can see that #item-body is empty.

Good catch. Looks like this error was introduced in r6517, when the '.php' extension was accidentally removed.

In 7230:

Adds missing .php extension when including request-membership template in bp-default

This extension had been accidentally removed in r6517, with the result that
the request-membership template was not loaded in bp-default.
